Case Management

Case Managers will work with all clients to identify problems and then develop a plan, seek, coordinate, and monitor services from a variety of resources, community agencies and staff. Case Managers are responsible for the following:

  • Identifying client needs and goals
  • Creating and implementing client service plan
  • Monitoring service delivery
  • Advocating on behalf of the client
  • Reassessing client goals and needs periodically
  • Collaborating with referral agencies to document client progress
  • Transitioning clients from formal support systems to self-sufficiency
