
Case Management
Case Managers will work with all clients to identify problems and then develop a plan, seek, coordinate, and monitor services from a variety of resources, community agencies and staff. Case Managers are responsible for the following:
- Identifying client needs and goals
- Creating and implementing client service plan
- Monitoring service delivery
- Advocating on behalf of the client
- Reassessing client goals and needs periodically
- Collaborating with referral agencies to document client progress
- Transitioning clients from formal support systems to self-sufficiency
